Content Request Form Creation - Step 1: Form Name & Instructions

The first step in the form-creation wizard requires you to give your form a name and provide some instructions to help requesters complete the form successfully. The data you input here will be displayed at the top of your form (example below).

To aid in helping requesters provide the level of detail you need, be thoughtful about your instructions. Encourage them to complete as many fields as possible and make note of any fields that are required. Finally, thank them for their contribution if appropriate.
